I have never seen holes worn through silt fences before, and I've seen some nasty sites. Maybe they should have used the orange fencing instead.
The only reason these fences stayed up is because they were braced against the concrete culvert

And what washed out through this culvert?

Well the orange porous fence held up fine!  No outlet protection, insufficient conveyance channel protection
Note the bulging from the sediment that was unable to bypass this incomplete silt fence
